Through our Short-Term Individual Disability policy you will gain peace of mind in knowing specific expenses will be covered should you become disabled.<\/p>\n<h3 class=\"wp-block-heading h2\">Short-Term Disability<\/h3>\n<h4 class=\"wp-block-heading\">Amalgamated\u2019s Robust Short-Term Disability Policy Offers Outstanding Features,* Including<\/h4>\n<ul>\n<li>Your choice of elimination and benefit period to meet Your needs<\/li>\n<li>Portability so that if you change jobs or leave your employer, the coverage goes with you<\/li>\n<li>Guaranteed renewable coverage to age 72 (up to age 65 in MA)<\/li>\n<li>Coverage Type\u201424 hour<\/li>\n<li>Disability payment amounts based on your income<\/li>\n<li>Maximum Benefits\u201460% of monthly salary up to $5,000<\/li>\n<\/ul>\n<h5 class=\"wp-block-heading h3\">Value Added Features and Policy Provisions<\/h5>\n<ul>\n<li>Definition of Disability\u2014Totally Disabled or Total Disability means that You are under the Regular Care and Attendance of a Physician and that for the first 24 months of Total Disability, You are unable to perform the Material and Substantial Duties of Your Own Occupation due to Sickness or Injury; and You are not engaged in any other occupation.<\/li>\n<li>Definition of Partial Disability\u2014A Partial Disability Benefit will be paid if you become Partially Disabled due to a covered Accident or Sickness. To be considered as Partially Disabled, You must first be Totally Disabled through the entire Elimination Period and have received at least one full Monthly Disability Benefit. The Partial Disability Benefit will be equal to 50% of the Monthly Disability Benefit.<\/li>\n<li>Mental Illness Benefit: Up to 3 months of benefits paid (not limited in GA and VT)<\/li>\n<li>Alcohol or Drug Benefit: Up to 15 days in any 12 month period (not limited in GA and VT)<\/li>\n<li>Waiver of Premium Benefit\u2014If You become Totally Disabled due to a covered Accident or Sickness, Your coverage will be continued for this policy and all attached riders without payment of premium. Waiver of Premium will begin on the later of the next premium due date following: <br \/>1. Your satisfaction of the Elimination Period; or 2. 90 days of continuous Total Disability.<\/li>\n<li>Pregnancy Benefit\u2014Total Disability resulting from pregnancy, complications of pregnancy or child birth is covered the same as any other Sickness. The Elimination Period for the Pregnancy Benefit is calculated from the first date that a Physician diagnoses Total Disability. Total Disability as a result of a normal pregnancy is not covered if the Total Disability begins during the first 9 months following the Effective Date.<\/li>\n<li>Accelerated Benefit for Terminal Illness\u2014We will advance to You the remaining months of the Monthly Disability Benefit payable to You under the Policy in a lump sum payment (not to exceed a maximum of 12 months or your Maximum Benefit Period \u2013 whichever is less) if You are diagnosed with a Terminal Illness. You must have satisfied your elimination period and are receiving Total Disability benefits under this policy.<\/li>\n<\/ul>\n<p><strong>Pre-Existing Condition<\/strong>\u2014means an Injury or Sickness for which, during a twelvemonth period immediately preceding the Effective Date of this Policy, You have received a diagnosis or advice from a Physician and received treatment, incurred medical expenses or taken prescription drugs. The term Pre-Existing Condition shall also include any condition which is related to any such Injury or Sickness.<\/p>\n<p><strong>Pre-Existing Condition Limitation<\/strong>\u2014means the period of time (as shown on the Policy Specification Page) during which no benefits are payable for a Pre-Existing Condition.<\/p>\n<h6 class=\"wp-block-heading h4\">Available Riders**<\/h6>\n<ul>\n<li>Continuing Disability Benefit: Intended to supplement a State Mandated or Employer provided disability benefit ranging from $100 &#8211; $3,000 per month.<\/li>\n<li>Physical Therapy Benefit: Pays $50 for each, up to ten, physical therapy sessions attended by a claimant receiving disability benefits under the base policy.<\/li>\n<li>Catastrophic Loss Benefit: Pays a Catastrophic Loss Monthly Benefit if the Insured: (1) has a Catastrophic Loss (due to an injury or sickness, the Insured is continuously not able to perform two or more Activities of Daily Living), (2) satisfies the Catastrophic Loss Elimination Period (90 days or 180 days), and (3) is under the regular care and attendance of a physician. The Catastrophic Loss Lifetime Maximum Periods are: 12, 24, or 36 months.<\/li>\n<\/ul>\n<p class=\"footnotes\"><em>Policy Form AMINDDI-10* <br \/>*Features &amp; form numbers may vary by state. <br \/>**Additional premium will apply to base policy.
